FYI there is no issue with the center of the fish it's just a glare but his face looks like it's losing color

Do you use a ground probe and/or GFCI on your tank?

Unfortunately, I can't tell anything from the picture. Let's bring @4FordFamily into the discussion if he is around. He has forgotten more about Tangs than I have ever learned.

That looks to me like HLLE.

Feed red nori, green nori, and soak a varied diet of drained frozen foods in selcon and over a few months they will likely heal up just fine. Maintain great water quality as that helps as well.

Do you use carbon? It's been discovered that it likely contributes to this as well. Hippo are particularly susceptible.

The picture isn't very good, so there may be something else going on there but for sure HLLE is present.

Yes. That's HLLE for sure. A pretty bad case of it. he can come back from it but he may always be a bit scarred from it.

Any other tips out there that can help me get him back in shape ?

pretty much just what's in the link posted above. Great water quality, low stress, excellent nutrition. High quality carbon as well.

If you do run carbon, make sure it is tightly packed so it cannot grind against each other and create dust.

There is some anecdotal evidence that stray voltage can cause HLLE. Do you run a ground probe in your aquarium?
