What's wrong with my favia

my favia started looking like it was melting or skin coming off around 1 of the eyes yesterday and now today it's moved onto 2 or 3 eyes. Nothing has changed and it's been doing good since I got it about a month ago

I feed reef roids 2 to 3 times a week and it's tentacles came out....was doing great up in till 2 days ago and looked like white mucus was on it or skin falling off so I've been blowing it off with a baster
