What's wrong with my fish?

Anyone no what's wrong with my Engineer Goby? Here's a video not the best quality. Plz help! He's been under the same rock for 3 months and came out yesterday and has been out ever since. Every once in awhile looks like he just is too weak to swim anymore and gives up and the current blows him into everything.

They're cryptic cave fish and avid diggers. Is he staying under a rock until feeding time and moving around some sand?
 
Not good.

Can you try a freshwater dip? Dechlorinated tap water is fine.....must be equal to tank temperature, so floating the container in the tank is smart, if possible.

At least 5 minutes....no more than 10-15 minutes if he seems to be tolerating it.

See if anything comes out in the dip water.

See what you get and we'll decide where to go next.
 
5 min freshwater dip done! What am I supposed to be looking for in the dip water. I see a little bit of what looks like sand.
 
That looks like sand that he may have had on him or in him. They tend to get it everywhere.
I'm no expert but something looks "off" on the shape of this little guy from the top down. I had one for awhile before it outgrew my tank and his head didn't look that triangular from the top down.
 
He looks very thin. You are sure it is eating? Mine is very fat and always under rocks except when I throw food in. My guess is something is going on. Does not look to be an issue with anything in the dip. Look at it with black paper under it, oops you had paper under.. Flukes will be white and a bit elongated.

Any new fish that may be bothering it? Any new corals or rock that may have a hitchhiker?
 
I have added a bta last week and a couple of torches. Will do a new freshwater dip this afternoon with tap water.
