What's wrong with my royal gramma？

I have a 25 nano. It's up running for a year now. Recently I removed three fishs out from this system to the new tank so he is the only one in this nano right now along with some corals. My royal gramma usually hides in his cave but yesterday he went out and laid on the sandbed. He's not eating at all. I did not see any white dot on him. He breathes really slow and not move much at all. His head turns whitish. ( could he be starving? ) I did dose hydrogen peroxide every day try to get rid of some dino( I put brand new carbon in already ). Any idea what's wrong with my fish? Thanks

Fish appears to have both ich and lymphotocitis. Its a virus and Infection appears on the infected fish as one or more white or beige colored pebble or wart-like nodules most commonly seen on the fins, skin, or gills, although other tissues may be affected.
What can you do? Isolate the infected fish in a Quarantine Tank and Feed it a healthy diet, including supplements such as garlic extract and selcon vitamins. Treat it for secondary infections using melafix or maracyn

That looks kinda like brooklynella. Does the fish almost look slimy in person? It’s a little hard to see for sure from the pictures.

O.K., I just wanted to check to ensure the dose was o.k. for fish - it seems to be.

Difficult to say what's going on from the pics. The second picture looks pretty good. I can see some spots on the other two pics, but nothing conclusive. Often, fish have more than one problem going on at one time. It could be Lymphocystis, royal gramma are one of the commonly affected species. However, that won't cause a fish to stop feeding, and you almost always only see it in newly acquired fish. The last pic you posted shows some sort of diffuse skin issues, not spots of ich like one would expect. Just to rule this out: could the gramma have dashed around and hit the rocks while you were catching up the other fish? That could cause diffuse scale issues like that, as well as the changed behavior you noted. That said, if you caught up the other fish more than 2 days ago, then that isn't it.

Thanks a lot for helping. It looks fine from the appearance except the whitish area around his head. The thing worrying me the most is he is not eating and wimming at all. He is more like crawling on sand. He's not interested in any food. He was laying against some corals. Those might stung him as well. ( He breaths really slow and steady and no slimy skin. I did a fresh water dip just now)

Update on the situation? My gramma is new and just lying down in the sand but he doesn't appear to have spots or miscolorings. I'm worried.

Hey sorry to hear that. Is your tank new? Mine still hanging there but he is still not active at all. Sorry I can't provide some useful info for you.

My tank is about 2 months old and literally just now my gramma swam to hide under some rock. I hope this is a good sign. Have you tried to medicate the gramma? I got a watchman goby last week who was sick. He died a few days ago and his head and tail turned white and so were his eyes. He stopped eating and became blind I think. He passed right before the meds came in the mail. Some of that info might be useful if it gets any worse maybe.

I don't know he's eating or not. He is the only fish in that 25g now. I still feed that tank every day. He is hiding in the cave and digging sand every day. Is there any possibility a bristle worm jumped on him? He still seems normal to me from the outside and no string pops as well. So I just leave him alone now. Thanks for the reply

Do not fear bristle worms. Bristle worms are detritus eaters.

Sorry for the late reply. I did not medicate the fish because I didn't see any signs of disease. I think it's better to treat yours since the watchman goby died recently but it's better to figure out what happened to the watchman or royal gramma first.(is the fish breathing fast?)

The gramma is out and is extremely active, I guess he was just shy. The watchman goby was sick when I got him I just didn't notice the signs until it got worse. He was breathing heavy and had a seizure I think and died when I quarantined him. However, the fish in my tank are thriving so I don't think whatever the goby had spread throughout the tank. Really sorry to hear that the your fish hides all day. Nothing is worse than spending money on a fish that you do not see often.
