any ideas what is wrong ?
 
Is it a new addition, or maybe getting bullied by another new addition? If so, keep an eye on the aggression and it will probably sort itself out in a week or two as the pecking order gets settled. If it's getting bullied chronically you may need to look at other measures in dealing with aggressive fish. In the meantime, just keep an eye on the fish and watch for signs of infection. If he's healthy he should just heal on his own, but if he gets an infection you may have to remove him and treat in a hospital tank for whatever it is that pops up.
 
Looks like aggression or scrape. Melafix should help the healing process
 
Could this be Flukes?
The tail being a bit tattered has me leaning more towards him being beat up than something like flukes. But if you don't think anyone has been beating on him then maybe you could try a freshwater dip or something to see.
 
It was definitely flukes. I just freshwater dipped him, and a bunch tiny white scale looking things have come off him.

Ps. I basically had to rearrange and take out all my rocks... to get him out of the tank.
