What's wrong with my yellow tang

I've included two photos and a link to my build thread, just go to the last comment and all the pertinent info about the tank is there. He has been in the tank for about 6 months. He has grown some but not a lot. He behaves normally, swims everywhere, eats like a horse and even comes to the glass when he knows its time for food. I feed mysis shrimp or plankton frozen in the morning and a 2"sq of algae and flakes in the afternoon. He has white streaks above his lateral line from his tail fin to just before the top of his head on both sides, they are identical and he has a white patch above his left eye. His eyes are very bright and he is active like he should be and gets along well with the other fish. I hope you can see what I'm talking about in the pics. Thanks for any help you can give me.
