No, it looks like it's flat, almost like his lip got peeled back

The reason I asked is sometimes we get parasite isopods, they can resemble pill bugs with little black eyes. They attach at the lips and gill plates.

Okay, with that said, it's most likely damage or injury. Catch the fish and QT it.

Tough to tell but could also be lymph. How long has it taken to get to its current “severity”? The timeline may help us here.

Treatment with anything?

The faded colors make me think something else may be up, such as in infection as @Flippers4pups suggested.

True about possible lympho, but with that there isn't much you can do for it.

I found a photo from a year ago. His colors are basically the same. The bottom lip seemed always big, whiteish. Well, in the past 6 months anyway. The first pic shows normal lips. The second pic was 6 months ago or so. Blurry, but shows the bottom lip is bigger. I thought it was normal as he matured.

Hmmm. Early diagnosis is important but also quite difficult.

I am Back to @Flippers4pups suhgesyion of a parasite of some kind (not ich or velvet) or lymph.
