What's your "this fish is suddenly SICK" response?

So today my white pygmy file fish was in a corner next to a hanging basket I have for baby rock flower nems. I was a little concerned because it was the middle of the day and it was in his 'sleeping' position (he was anchored onto the basket). A few hours later and hes flopping around in the current of my tank before settling on the bottom.

What would you do in this situation?

He seemed fine yesterday and always eats. About once a month he gets a white spot that moves through his dorsal fin. I've had him over a year and he's captive bread. I QT all my fish before introduction (20g tank with the pygmy, a clown, and a starry blenny). I have not introduced new fish in over 8 months.

I inspected him and I see no white spots in fins or otherwise parasitic signs. His lower fin is a little bit rough around the edges but nothing substantial that would make me think he was attacked.

I tried to set up my 5g QT but I spilled salt water all over my kitchen and now I don't have enough to refill my display tank (QT tank about 1/3 full). I thought I'd try to make it work at that water level but then I dropped my spare heater and shattered it. One of those nights I guess. My usual LFS isn't open Wednesdays due to the human quarantine so I'm probably out of luck until Thursday.

I currently have the file fish in the display tank but he's netted. I propped it up to the glass and bent the wire handle so he has a place to rest that's near the surface without getting pelted by high current. His eyes are very responsive when I come near. He isn't scared of me. I could hand feed him and he would even swim around in my hand and let me scratch him (I never did this often).

I added 4 emerald crabs and a Space Invader 2 weeks ago. 2 of the emeralds died within a week. One I caught munching polyps off a leather and is currently in a floating basket. The other is behaving/evading capture.

Were the new additions QT'd?
The coral had a 15 min Coral Rx dip and 15 min iodine dip. Crabs were observed for 48 hrs before adding them.

Encysted parasites could have come in with the new additions.

I'd do a freshwater dip. And check for flukes. If it's velvet or ich, the FW dip will buy some time. Just be mindful that there's all ways risks with any treatment.

And then a bath in Ruby Reef Rally for 60-90 minutes and then into copper or CP to treat for parasites.

Hopefully it's just that but I think it's wishful thinking. I've never had a fish recover from something that impacted it's swimming. If he dies I'll most likely QT the other fish and medicate with prazi and food treated for internal parasites.

How would you recommend matching pH for the FWD? I'm not sure I have anything on hand that's specifically for pH.

The dip is so short, 5 min. max that pH is usually not a problem.
 
Gave him a FWD last night. He seemed more active afterwords but I kept him in Net in the display tank. Net was positioned so he could get out if he wanted to. He’s MIA today so I think he died :(

EDIT: Found him. An emerald crab had his body tucked away in a hole. Had to pull apart rock work to get the body. RIP
