No clue why but my coral beauty does this all the time over and over again. Any clues why?

I see this most often with flukes followed by ich and velvet. It's one of the signs to me that requires urgent intervention. Some great articles over in the fish disease section that will be helpful. I would recommend starting with a freshwater dip to identify flukes, and determining further course of action from the results of that.
