Why is my angel looking like this.

I don’t know what It going on. Do any of you? There are spots that used to be blue that are now gray. His forehead is also grey. Sorry the picture is bad.

It looks like a stressed fish with a slight cut on the dorsal fin, how long have you had it?
 
His tail is also ripped. The tank mates are a six line wrasse, a pair of clowns, and a skunk cleaner shrimp. There are also a few corals but none could stress him out.

+1 to bullying, the number one suspect would be the sixline. These aren’t sweet unless you have a tank dedicated to them with more rock than usual.
 
So how could I prevent this? There is enough food going into the tank and there is lots or rock and space for them to hide. The parameters also stay in good condition since we Change water weekly. Anything I should do?

You can try rearranging the rock-work in the tank but in all likelihood someone will need a new home.

You’ll have to give away one or the other… if you keep the wrasse you won’t be able to add any other fish.

It’s diet is seaweed, spirulina, brine shrimp, and saltwater spectrum pellets. The tank size is 21 gallons of swimming space with a 4 gallon sump. The sump has a skimmer, and I have carbon and nitrate and phosphate remover running. I also have bio block and around 7 pounds of rock running.
 
i would reconsider not removing the wrasse as this is the likely scenario for any fish you add moving forward if it stays. Melanurus are a very nice looking, considerably more docile and relatively inexpensive wrasse that will cover a lot of the same bases as your 6-line if you needed a fish to replace him with.
edit: i just noticed your display is 20 gallons which realistically is too small for the angel, 6-line or the melanurus i suggested. In a tank that small with nowhere to hide its not going to end up well in its current format
 
$20 on the 6-line. They can get nasty.

If you’re sold on having a wrasse, how bout a pygmy possum wrasse? Much better for that tank size as well.
 
We have decided to move the wrasse to another tank. It’s a 40 gallon with no other fish. Any tank mate recommendations that wouldn’t get bullied by the 6 line? Thanks for your help everyone.
 
This fish is a product of aggression and also seems to be a little thin at the head area.
Your diet is inadequate for this fish. Add foods such as :
mysis shrimp
LRS fish frenzy
SFB Angel formula
Marine cuisine
Formula 2 frozen and flakes.

Add selcon vitamins to foods occasionally. You can also place the 6 line in a cllimation box allowing the cherub to heal
