update--never mind....this was a pre lights on post... lights now on and no spots. lol.

5 year old 150g mixed reef.
no disaese issues for well over 1- 2 years
live stock is
4 chromis
1 yellow tail damsel
1 female anthis
1 engineer goby
1 tomini tang
1 yellow goby
alot of snails
several urchins

added a powder brown tang yesturday hes the biggest fish in the tank. this morning 3 white spots on him. WHAT. yes it was qt for 3 weeks in a corner room in a tank that was 4 feet from the ground.... lol.. im not wanting to discuss a qt method and process.

Added a new unquarantined fish? Just venting or wondering how this could happen?
 
omg really hahahah -- yes it was qt. i figured a five year old tank would tell people that a person qts, lol.. i will add that to the post.
 
If you didn't QT the fish before adding it to your DT, you have your answer. If you did, and have had ich in the past, you likely still have it in your DT. While your existing livestock is healthy enough to consistently fight it off, the new addition, likely stressed from the transport chain, to your QT and then to your DT, is not. Tangs (Acanthurus especially) are highly prone to ich. If you can, pull it out and QT / begin treatment immediately. If that is not a possiblity, treat the entirety of your tank with Rid Ich by Ruby Reef. The latter option is probably more preferable, given that your existing specimens may likely be stressed by the new addition, too, making them susceptible now, as-well.

Let's hope it is, in fact, Ich, and not velvet. Though, to be fair, I may have jumped the gun without a proper diagnosis. Can you provide pics to confirm?

Will be hard to assess a fish and what it has without a pic or two under white lighting for clarity. Can be an array of issues

Cannot make assumptions.

What is your QT process?
 
Your display could be “managing” ich and it only shows with new additions.

No pics. No info on QT methods used. Just what exactly do you want to discus?
EDIT : Also no history on the 5 year old DT. Just because it's 5 years old doesn't mean it's disease free.

Not seeing the spots, I can't say for certain, just be aware that it is normal, at the start of a marine ich infection, for the trophonts to drop off the fish during the day. They then release theronts, usually over the next 4 days or so that re-infect the fish. This results in many "my fish are cured!" remarks, followed a week late by "Oh, the ich returned" laments...when actually, it never truly left.

That said, sometimes spots are just sand grains or bits of mucus.
