Wound or something else on tang?

My blue eye tang just had these marks on his head one day. It wasn’t a slow process, literally overnight. I assumed it was an injury from fighting or rocks or who knows what. I thought it would just heal but it’s been a long time (months), and no change. There’s a little chunk of flesh missing in the wound and I want to say I see something at the base of it, but it’s hard to say for sure. Fish seems happy and eats fine. I’m starting to wonder if it’s a parasite but I can’t get good enough look. I’ve also wondered if it’s a form of HLLE but I’ve experienced that beforeE It’s on both sides of the head but slightly different on each side. Attached a pic, what do you think?
