Wrasse crazy

Hes pretty new in the aquarium...
Hes always hitting that same rock
 
Almost appears to be scratching or flashing against the rock as if he has an irritant in his gills. (Ich and Velvet both like to start in the gills where the white spots aren't present.). You said he is new. Did you QT him?
 
he did however changed a lot of his color in the past 2 days turning into darker green color
 
Wrasses can be "fluke" magnets. First thing I'd consider doing is a freshwater dip to either confirm or eliminate this common problem.

Do you have an anemone in your tank? I caught my melanurus wrasse trying to get pierces if reef frenzy away from BTA and paid for it . He would scratch his mouth or ram rock lol after he realized what he shouldn’t be doing ! Last time I seen him do either !
 
I just introduced my fairy wrasse to my main tank last week. He does not flash against rock like that, but he does swim up and down the back end of the tank, doing loops around the rock work there. I also think he's crazy. Can't get him to stop.

My other fish flash against rock or sand like that once in a while. I believe it's a sign of them marking territory, not necessarily scratching themselves. But I would just monitor to see if health declines.
 
I have a 1 Majano wich I will be getting rid of soon , a couple aiphtasias too ...
I have another wrass in there wich has been in for longer so i'm pretty sure it's not a problem native to my system.
I have many corals and maybe a higher than normal amonia in the system, i'm up for a water change like tomorrow.
I want to add that he is not doing that all the time (it's been 3 times i've seen him) and it'S always on that same exact rock.
I actually got lucky to film him.
But I already lost a wrass back in the days to a gill fungus so it does stress me :(

You need to take the wrasse out to do a fw dip.

If he is doing it on the same rock I suspect he is just marking territory as I mentioned in my post above. Just monitor for now and see if his condition changes. FW dip can be stressful on fish.
 
LOL I know what's a freshwater dip, I Mean How do you catch a Wrasse in a full tank display?
 
But if it is a parasite , can it go away by itself or the fish will most likely die?
And can I add a cleaner shrimp to help his condition ? Or any other help ?
 
If it is a parasite, it most likely won't go away by itself. Cleaner won't necessarily help. Depending on what the parasite is, you may or may not be able to treat inside the tank.
 
Then Ill start looking for some of those traps ive seen a nice one that acts like a bubble , when the fish is inside it turns and floats back up lol

He is great , looking greener and greener , I have been told it is a red line wrass ... i'm starting to think it could be something else.
I havent seen him bend himself in pain or itching for a couple days now,
no visible spots on his body , breathing seems normal and the fish is very active :)
I wonder though if he might have given my clown a beat up. My clown use to be territorial against him very close to that rock he bounces off and now my clown have some colors problem ....
still monitoring!
