I had a six line wrasse in QT for about a week. I wasn't treating with anything, just observation. Today it was lethargic, not eating, and heavy breathing, it didn't make it. I have another fish in the QT tank that seems to be doing fine so I'm trying to figure out what I may need to treat for. It looked to have some jelly like spots on its body.

I agree wrasse don't so well in barren qt or hospital tanks.
They need rock. Sand. Flow. Foods. Ect. An environment so to speak.
There's a wrasse expert on here. Hopefully him or her chime in on the subject.

Sorry to hear. Wrasses and other do well in quarantine provided with observation they have non preexisting symptoms
Wrasses are susceptible to flukes and if no medication such as ruby rally or praziquantel is in the water, through stress alone, anything can pop up on a given fish

I have put several wrasse through the full QT protocol (Copper Power and Prazi) with no problems. I always put a plastic tray with about 1 - 2 inches of clean non-calcareous play sand in the QT for the wrasse to use for sleeping and hiding. Otherwise, bare bottom with some PVC pieces and a top to catch jumpers.

Welcome to Reef2Reef, sorry to hear.
Observational quarantine is always risky because symptoms of disease can be missed, and the if seen, the disease has already started, so you end up chasing after it.
The clear things you saw may have been Neobenedenia flukes.

Thanks for all the replies. I also have an algae Blenny that was in the same tank. Should I freshwater dip it and run prazi? I already had a water change planned for today, should I just break down the qt tank completely and start fresh?

You could try a 5 minute FW dip and then look for flukes. If any are seen, you could then do prazi.
