Wrasse has an open sore. Please help!!

I don't know how it happened but it does not look good. What should I do to treat it and what could have caused it?

Wow that doesn't look good unfortunately. Possibly a coral sting? I would try to get him out and treat with Kanaplex,metroplex and Furan in a QT. If you cant get him out you could add metroplex to your display tank which in my experience is reef safe.
 
Looks like an injury. You do not want it to get infected. Two options, get into a qt (hospital tank) and treat for antibiotic infection or leave and watch. If worsens, get him in hospital.

So I have decided im going to set up my hospital tank as soon as I get home from work. What medication should I order for treatment?

So MB First.

Then QT tank, I think @Humblefish will agree as I recently used the "trifecta" with a bad infection on a Blue hippo for @4FordFamily, upon his recommendation.

The Trifecta is a combination of 3 meds. Metroplex, Kanaplex, and Furan-2.
Metro and Kana get dosed every other day. For at least 10 days
Furan-2 gets dosed daily for at least 10 days.

Follow the instructions on Furan-2 packaging for WC Schedule.

Make sure to treat for the 10 day period even if the wound/infection begins to heal and look better prior to the end of the 10th day.

Also I don't know how many gallons your QT is, but the furan-2 comes with 10 packets per box. Each packet is good for 10 gallons. For instance the fish I was treating was in a 55 gallon QT. So it took 5 packets per day for 10 days. meaning I needed 5 boxes. Just wanted to make sure you knew that. The metro and Kana should have enough for 10 days as long as you are treating a smaller QT.
 
My QT is 20 gallon, I will buy 3 boxes in case I need to treat for longer than 10 days. I really want to thank everyone for your help.

Another thing I might add, anytime you are medicating a QT. The meds deplete the water of oxygen. Highly recommend an air stone in combination with a power head aimed upward at the surface of the water, to help with this.

That is what I would do, it also leaves you a small amount on your shelf, if needed again to at least start until more can be ordered if a future need may arise.

One more question, is it going to be a problem that there will not be sand for him to sleep in? or do you think I should add sand in a Tupperware?

Totally fine, just pull a small amount from your DT and put in a Tupperware. Then dispose of it when you get him moved back over.
 
Ok I will do that. I already ordered the meds and they will be here Wednesday. I am going to wait until tomorrow to catch him that way I can give the sponges im going to use in the power filter some time to populate some bacteria. Thanks again to everyone.
 
So unfortunately I found the wrasse dead this morning, very sad. The wound was much worse this morning. Here are some close up pictures.

Bummer, sorry for your loss. Major infection you had going there.
 
This same thing has happened to my yellow and purple wrasse but he made it and it wasn'tso bad, so you think there is something in my tank causing this?
