Wrasse not looking so good... help

My wrasse is not looking so good and Scales are changing color, has cloudy eyes and there is a white string looking thing coming out of his behind. He is not swimming just laying on one side at the bottom of the tank. Breathing very heavily and is getting picked on by the rusty angel fish. Any ideas on what’s wrong?

Sounds like internal parasites have caused a bacterial infection (cloudy eyes). Has the fish been dewormed?

No I have dewormed, never had any issues with fish. My tank has only been up and running for 8 mouths. Could this spread to other fish?
I have PolypLab medic for ich if I ever got it.
