Yellow Assessor Help

Had this fish for over a year and was always out and about and a very bold eater. Over the last week it hasn't eaten much and spent more time hiding in its cave than usual. This morning I noticed this circular patch over its head. Does it look like a bite mark?

If it's hiding something is bothering it...stray voltage?

Doubtful. I think it may be getting bullied hence the circular patch right below its head. Cant tell if it's a fish bite though.
 
Need a better pic. What are it's tank mates? Do you have a canopy or cover.

Yes have a cover. But definitly not from that. 2 skunk clowns, linear blenny, and a firefish.
 
Can't say I know. However, I will say that my yellow assessor is very reclusive. It will be several days before it pops out and visible. Other than that it is hidden under its favorite rock and structure. Then there are some days that it says hey, here I am. All is well then poof, another week until I spot it.

Not really helping you I know but just saying how mine is. If you are able to get a better picture maybe others can help. Other than that just make sure food is making its way to its location so it can eat.
 
It's not really eating either which is a concern also. Hope it's not dying.
