my yellow wrasse has been showing some discoloration, splotches of white over the last 2 days. Not sure if this is from rubbing on rocks or disease???

Looks like scraping/scrape injury. Is this fish rubbing itself against sharp rock or objects in a twitching manner ?
How is breathing rate - Normal or labored?
Any aggression/chasing you've noticed ?

He looks perfectly normal. Breathing normal. No shaking, no rubbing. Eating great.

Haven’t seen any aggression in the tank
What other fish are in tank with it?
Does fish hide in a certain crevice where it is scraping itself to get in or out ?

1 blue hippo
1 jaw fish
2 small clowns
Clowns are my suspect. A sure way to find if so is to place your phone on video mode for about 40 mins and drop a little food and walk away- YOU MUST WALK AWAY
Come back and review video and you should see your culprit. Aggressors usually bully others when food is present

I will try that…

BTW - All of my fish were purchased at TSM. hopefully it’s not a disease / infection Issue.

How long has it been n your tank? The tail sure looks like some other fish has been biting it. You may never see it happen, so you look for evidence like this.
