Yuma needs help

Hello,

I picked up a nice Yuma frag about 2 weeks ago. Placed it on sand bed, looked great at first, but started to look like it was stretching off the plug after a few days. I moved to a lower flow area with less light intensity. Now it looks like the mouth is falling apart(actually watched a piece fall out...curly piece just left of the coral edge). I'm pretty sure it's on it's way out, but am holding out a bit longer! Thinking of trying to isolate it in a shot glass or something. I've included a pic of a Rhodactis doing well 3 inches away. Thoughts? All other corals are thriving and actually sprouting a lot of new growth :) Tank is 13 gal LPS, 5 months old. Thanks for any help.

Could be still acclimating. Mushrooms are weird. They can look dead and have no life to them and the next day they’ll be happy
