Zoas seem to be wasting away!

My zoa was purchased about 4 weeks ago and it was a full frag. But now I noticed part of one side wasting away. I did a freshwater dip with iodine last night for about 1.5 minutes.

Has anyone else had this issue?

Any advice is appreciated?

What’s your tank parameters? Never fw dip corals.
 
Yea you probably shouldn't have done a freshwater dip. Can you see any pests? Do you have Coral Revive?

Ph 8.4
Am 0
Nitrate 10
Nitrite 0

I thought I saw a tiny spider but it was hard to tell for sure what it was?
No, I do not have coral revive.

Found two of these white worms in my tank today not sure if they are the ones doing damage to the zoas?
