Anyone ever have a fish choke to death?

Started using some rods reef food about 2 weeks ago. Never thought something like this would happen.

Fed tank as normal this evening, walk by a couple hours later to find this guy on the bottom of the tank. Have had him for 4 years.

Yes. An Oscar choke on a fire fish in my old fresh water tank.

I've almost had this happen, in a frozen food blend, a piece of a shrimp exoskeleton got stuck in the mouth of one of my chalkbass. You have to be careful with rods / reef frenzy for this reason.

Same thing almost happened to my clown. I was running low on LRS (all I have ever used). Pulled a sample bag of Rods out of the freezer and thawed a piece in tank water, smashed with my fingers and poured it in the tank (just like I always do with LRS fish frenzy). Immediately saw a couple large pieces of shrimp (1/4" diameter and 1" long) and scooped them out with a net. Then I noticed the clowns was swimming with her mouth completely open and could see a piece of food. Kept an eye on her and she was finally able to swallow it, but I will be keeping an eye out for the larger chunks.

Yes completely thawed out, and thought I removed all the large pieces as well. Guess not. Won't be using anymore of this food. Not saying it's a bad product but don't want to risk it with the rest of my livestock.
