Would hate to lose this guy as he was a large investment. Other clown is fine. 55 gallon, no changes in parameters. I have a sea apple and urchins, all fine. Been in tank for 4-5 days. Drip acclimated for 2 hours.

Kinda hard to tell from a pic, but are these white spots on the guy or just bubbles in the water? He looks thin but that wouldn’t be an acute prob.

Do the fresh water dip and watch how they fall out
 
They are bubbles in the water. Brought him up to the surface in a soft net while i wait for the water to warm up. He seems to move a little bit. I noticed he was skinny too but I have been feeding mysis every day and he seemed like he was eating
 
They look like white spots to me.
That plus gasping could indicate ick or velvet.
If you have QT, I’d pull him, but just him until we can get an idea what, if anything is going on....
 
When I put him back in he started swimming at the surface and poking his nose through the water then fell back to the bottom
 
What's your pH? Does an airstone help?

Could you provide a list of all inhabitants?

Aren't sea apples and sea urchins prone to excreting toxins into the water when unhappy?
 
Tank is 6mo, sea apples eject their toxic organs from their body under extreme stress. If that was the case I would have no fish. Livestock is as follows:

Pink spotted watchman goby
green mandarinfish
sea apple
2 sea urchins
maxima clam
peppermint shrimp
cleaner shrimp
coral banded shrimp
black storm clown
