How long after starting treatment will it be before I start seeing relief from the scratching and redness?

That can vary. If it is flukes (no guarantee on that, it just points in that direction) you typically a relief of symptoms by 4 days.

So, today is day 6. Yesterday evening she was scratching like crazy. She still has the red rash on her back end too.
How long do I go before I need to consider it may be something other than flukes?

You always need to keep that back of mind. Diagnosis, especially remote diagnosis like we do here is never 100%. The key thing is : is there compelling evidence that indicates a different treatment should be attempted? I don’t know what that would be. Copepod micro predators can cause skin reddening and lots of scratching. Bacterial disease can definitely cause redness, but scratching isn’t a common symptom.
If the fish is eating and breathing o.k., I would dose with prazi again and then consider antibiotics. Conversely, you could dose with antibiotics now and redo the prazi if that doesn’t work.

Thank you. Her scratching today hasn’t been as bad. Still red. No heavy or unusual breathing but i would like to see her eat more, she only randomly picks right now. Since I am at 7 days tomorrow on Prazi and going to start my 7 days break period, how do you feel about me doing a round of Metro in case it may be bacterial or just in case she has any open wounds from flukes? I can do a WC before I do a second round of prazi in a week.

Metro only affects anaerobic bacteria, so won’t work in this case, since the red lesions are exposed to oxygenated water. Gram negative, broad spectrum aerobic antibiotics like Neoplex or Furan-2 would be better choices.

I have completed a round (1 week) of Neoplex with no change. I did 3 rounds of Kanoplex, finished it this past Saturday. she looked better. She is eating a lot more, always hungry. Did a 20 % WC on Sunday, replaced my skimmer cup and ran carbon in a bag in a sock for 48 hours, removed it last night. Today she is still hiding, breathing heavy and looking irritated again today. Going to try another 3 rounds of Kanoplex. @Jay Hemdal any other suggestions?

Did the breathing get better during the kanamycin and then ramp up again afterwards?
Any other fish in the tank? Are they showing symptoms?
I'm not sure what the protocol is for repeating kanamycin treatments - the typical dose is 50/mg/l every other day for three treatments.

I can’t say that her breathing improved, but her appetite did and she didn’t look as irritated. It almost looks like the redness at her rear is coming back some and then the HLLE look to her is making me think she is really stressed. She is still flashing but not presenting to the one cleaner shrimp I have left. I thought it was the carbon that bothered her but it’s been out 24 hours now. Could that be it?

I have actually been feeding her per your earlier recommendations. I am feeding;

LRS Herbivore Diet
Spirulina brine shrimp
Nori seaweed
Formula 2 flake
Mysis shrimp
Phytoplankton

while going through this I have been soaking food in both Selcon and Garlic Guard

Carbon can increase the chance of a tang developing HLLE, but it shouldn't cause any irritating reaction in a fish.

Thank you Jay,

Earlier in this thread you had mentioned that "Yellow tangs commonly show red skin due to a variety of irritations." I had received an ICP test back a couple of weeks ago and I had very high amounts of Iron and Zinc. Because I have been medicating I have only done two 20% WC's.

I have read that water quality is a major stress factor for YT's, could my issue be that simple? I am going to do another WC tonight and another in a couple days and see if that is the issue.

I have gone through 2 rounds of Prazi, feeding Metro, 1 week (1 dose) Neoplex in the water, 1 week (3 doses) Kanoplex in the water. No other fish are showing any signs of issue.

Are there other issues that can cause irritation in them?
