Everything was fine until 3 days ago and that’s when she started acting weird and shy always hiding and not eating hasn’t eaten in about 3 days. Only change to the tank is I added a new piece of live rock. This happened out of no where. This is what she does basically lays down and looks dead but is breathing. I have her in my refugium right now to give her space. Any ideas what happened?

What are your last known parameters? - this may help the more knowledgeable readers here pinpoint the problem fast. Whatever is wrong, it doesn't look like you have a lot of time to waste.

Parameters are good all zeros except nitrate 20 ppm. Had the fish 3 weeks. I do not have a QT it spent a week in my refugium loving it and then a week in DT loving it swimming around picking at rock and then boom

probably internal parasite. fish dont usually shy away and hide and stop eating unless sick. hard to say what exactly caused the degrading of health. if fish passes away you can place in freshwater to see if any worms come out.

Should I be worried for my other fish

Do you have other fish in this tank?

How long ago did you add the rock?

Did the rock come from another system, or was it dry?

i would run some prazi pro through the system specially looking at pics it doesnt look like you have much in it. Treat all incoming fish for internal parasites and some for external. QT is a good practice and will save you a lot of headaches down the road.
If rock came from another tank maybe it was used with copper. if so i would run carbon and poly filter.

And other fish I have a niger trigger, dog face puffer, Midas blenny, long nose hawk, 2 clowns

I second running prazi pro for internal parasites. Based on that picture, it's possible that nothing you do will help at this point (in fact, dosing medication may stress the fish further). Still, I would personally prefer to do something rather than do nothing.

Praziquantel has to be mixed with food for treatment of internal parasites, and digested. Easier to bind GC with focus into frozen food for treating of internal parasites.
Dosing it into water column will eliminate flukes etc.

While introduction of a parasite from the new rock is a possibility here, I would find it less likely to be the cause of death given its only been in your system for a few days. It is possible though if it were velvet. The fact that your other fish are ok is what makes this less likely.

The rock could have introduced something unwanted to your tank As mentioned above, Angels are very susceptible to issues related to low oxygen as well, or ammonia.
Do you have a sump? Skimmer? Ample agitation of water at the surface?

I have a sump, skimmer, UV sterilizer, refugium

Is this what I should dose with?

I would only recommend dosing if you can confirm what needs treated. Prazipro is helpful in treating worms and flukes etc. But if you aren't sure that is what you are dealing with I would not dose the medication.
