Found this fish in my tank, what is it?

Do you have a Cucumber? There's a type of fish that lives inside Cucumbers and occassionally show up in tanks...

Just googles a pearl fish, it does resemble that. I don't have any cucumbers in my tank though, I have two clams and have had them for a long time. Maybe it's been in there the whole time.
I thought it was strange finding a fish I never put in there, and it was weird looking too with those little eyes on top of its head.

Um... How big of a tank do you have that you havent noticed a fish that size until it was dead?

My tank is 150 gallon with over 200 lb of liverock, and I look in it everyday for probably at least an hour so it's difficult for me to imagine not noticing this fish (I even found and caught a sea spider, if you know how difficult those are to find). But none the less, I never added or seen this fish before I found it dead.

First thing that popped in my head was a goby as well, with the shape and eye placement. This was about an inch and half long and seemed to be orangish colored. It looked like a tadpole.
