The spot on the clownfish could be an infection starting up (or possibly a coral sting from an LPS sweeper.) I would just continue to monitor for now until more symptoms develop (if they do.) Clownfish are notoriously bullet proof. 
The white thing crawling in the sand could be any number of things... bristle worm, amphipod, etc. It wouldn't be related to what's happening with your fish though.
