I feel your pain. I just broke down my 250g tank 4 days ago. Had to pull all coral & rocks to get my fish out. Now my house looks like a LFS store. I have 2x 40g beeders, 2 x10g & 1x5g. All fish hit 2.0 copper last night so let the counter begin.
During this covid crisis I got the big Idea of adding a fish I've always wanted. Powder Blue Tang.... yes I know. Ick magnets.
I treated with copper but a made a miss calculation with 5 chromis I bought a couple of days after the tang to disperse aggression when putting it in DT.
I had the days wrong for how long they where in copper. Very big mistake!!! I use 2 40g breeders 1 with copper & 1 without. I only copper for 14 days, then put in clean tank. This gives no room for error. Now all QT tanks have copper and running it for 30 days. Can't put anything back in DT for at least 76 days anyway. Sick to my stomach about it!

The spots you are seeing may not be Ich. There is a gram positive infection that can look very similar to Ich. It usually presents after a fish has been in Copper. The only way to know for sure is to do a scrape and take a look under a scope.

You don't treat it with meds. It's best treated much like we do for Lymphocystis. Good water quality, low stress environment, high quality foods, paying attention to unique species requirements, and with vitamins to boost the immune system like Beta Glucan.And how do you medicate for that...?
So you didn't add any corals or inverts to the tank during those 90 days?
Or could the copper dosage been insufficient? I personally did TTM with all of my fish. I didn't want to bother with measuring copper. And now I have an Ich-free tank.

If you are sure you did everything correctly, do wait it out a bit to see what happens, it “could” be flukes or a kind of infection, both of which will clear out on their own if you have a few cleaner shrimps in the tank.
On the other hand, something could always have gone wrong with something left unquarantined or a piece of equipment, or copper or something and you may indeed have ick. I had to clean my tank from ich 3 times in a row since last September due to mistakes done by me without realizing them when I made them. But latter it all makes sense....
Anyway, for cleaning fish from ich and velvet, there is no more surefire way than daily tank transfer.Easy, sure and better for the fish than toxic copper
Regrettably I’m been told numerous times that some fish are just ich magnets. Tangs are very susceptible especially the regal and powder blue.
How true that is I don’t know but I know some hobbyists who avoid tangs full stop for that reason.
I know this doesn’t help with your current outbreak but thought I’d share what I’ve heard.
I have found that copper causes more problems than its worth.I have gone all natural with a mixture for four crushed garlic cloves and a one inch peace of ginger.Slice ginger in thin slices and add all to small jar and top up with R O water.let stand in frig for a couple of days if possible to gain strength Add a couple of drops on there food twice a day and things should get better in a few days.I have used this on my blue tang that seemed to get everything from ich to black spot and I still have the fish after several months and he is fine.Treated right in the display tank.Good Luck

I quarantine 4 tangs (powder brown, sailfin, blue hippo & bristletooth) for 5 weeks and they all came out with no ich. They have now been in my DT for months. The 1st week in QT I just observe them doing a water change afterwards. The 2nd and 3rd week, treat with cupermine and prazipro (following the directions) doing a water change after one week and re-applying. Carbon needs to be removed when using cupermine. The 4th week after a water change, apply paraguard for the week and do a water change afterwards. The 5th week is just to observe before transferring to DT.
