Could this be ammonia burn? I’ve never encountered it myself but I’ve seen a lot of pictures and it can look similar.
 
Wow I’ve never seen that. I agree with @Maritimer that i would treat with antibiotics. It almost looks like lymph and hypermelanization

Got it from blue zoo 10 days back. Was doing well until Wednesday. It’s in a QT with the rest of the shipment.

Other than a lack of appetite it’s doing ok. At first he couldn’t close the mouth but that seems to be easing up. Moving the mouth again now.

If you can get some more pictures, I'm sure everyone would like to see them. This is very strange. Agree with using the antibiotic trifecta for now at least.
 
I agree with the others.

I do think it's bacterial in nature. It's just not presenting itself as we would normally see.

One other medication to consider is Sulfaplex. It works well for infections on or near the mouth. You could use it instead of Metroplex if you wanted to along with Kanaplex and Furan-2.
 
No kanaplex or sulfaplex on hand. Second round of furan-2 ends tonight. Will continue the metroplex tomorrow.

Sulfaplex does much better with mouth infections. I’d recommend it’s use, we buy on amazon.
 
I ordered the sulfaplex and other Rx that I was out of so hopefully it will be here Wednesday. How long can he go without eating? He hasn’t eaten in two days.

Depends on the overall health of the fish. I personally had an Achilles tang in QT for @4FordFamily a while back that did not eat for several days due to a bacterial infection near the mouth.

Even once the infection was gone the fish did not eat like an acanthurus normally would. That fish is healthy and in his DT today.

So I know this answer isn't helpful, but 3 days all the way up to a couple weeks is possible depending on the health of the fish. I would remain hopeful at this point. Try offering several different food types. If you can get the fish even to take a bite or two here or there it will increase the odds significantly.

The issue is the infection near the mouth causes discomfort when eating, so the quicker you nip the infection the better off you are.
 
It almost looks like there is some tissue necrosis in that last picture. Fish can survive for quite a while without food if they start out healthy and in good shape. Hopefully what ever infection this is is sensitive to the abx!
