My Flame Angel has had that Popeye for a while but I don't think it's bacterial as it's just one eye and I've already done several Anti Bacterial treatments and it's still there.

However, he's developed this White bulge on his bottom. Any thoughts?
 
That looks bacterial to me as well. Lymphocystis generally looks like small/medium cauliflower lumps.

The eye may have initially been injured, but it looks to have progressed into a bacterial situation.

What antibacterial treatments did you use? Erythromycin often works well for eye infections, but I suspect this has gone beyond that. Furan-2, Metroplex & Kanaplex, used together can help to blanket a wide array of bacteria, and I've heard good things about Nitrofuracin Green - though I've never used it myself.

@melypr1985, @Humblefish - any suggestions?

I did a full dose of Fritz Maracyn. And I've done Kanaplex and Metro. He's currently in a Treatment of Kanaplex.

Bacterial infections are notoriously slow to respond in marine fish - treatment should be continued for a minimum of ten days, longer if needed.

Not 10. Guess I'm doing that then. I'm on day 3 of the Kana treatment. Should I add the metro?
 
Yes - and the Furan-2, if you can get hold of it. Follow the package directions for each, just as though you were dosing separately.

So run the triple anti biotic.... geez. I swear I've needed to run anti biotic on every QT I've done since I started QT'ing

It can often help to tear down the QT and clean it out with bleach & vinegar between rounds. That should help keep bacteria populations within reason.

Stuff happens. I agree with the three antibiotics mentioned.

Also, this fish looks pretty beat up -- it could be infection eating the fins but any tankmates?

He's with a Sohal and a Foxface. They pay him no mind. He got the Popeye right after switching into a new QT. So I think it started when he scuffed on something during the transition.

Yikes the sohal has me questioning... my sohal is pretty good most of the time but he can turn on a fish quickly. This fish looks like it's being picked in and it's also quite thin. Try feeding live blackworms -- some calories might improve the total picture for the poor fella.

Oh, I've had him for months, he's a good eater. He's also about 2 weeks off of CP Treatment for Brook and Velvet. So there's that stress as well.

This makes me wonder if he has internal parasites. I would source focus and metro and add it to drained fish food (by seachem) to treat for internal parasites. They're not uncommon in centropyge, and it should not look that thin if it's been eating. Blackworms are very nutritious, I still highly recommend. :)

I also cannot see the spot you speak of that you believe is lymph.
 
I think it could be flukes/worms causing some of the problems here. If the Sohal has black ich, then odds are it will spread to the other fish he shares water with.

I know you said you weren't comfortable doing a FW dip, but it's really needed at this point to confirm/rule out black ich. If none of the dark spots disappear following the dip then I think we need to focus more on an aggressive bacterial infection being at the root of your QT problems.

I always lost a fish after the FW dip..... always.....

Let us know the results of the dip.

For what it's worth about the antibiotics.... You'll want to follow the dosing directions as far as amount per gallon, but pretty much ignore the rest of the packages. You'll dose each to the tank (dosing each med a few hours apart is helpful) and do a water change 48 hours later, then repeat for 10 days minimum. I hope that helps clarify :)
