I just picked up a Naoko Wrasse Thursday evening and put into QT. He ate a little the night i put him in and was a bit skittish which i thought was normal, but all yesterday and today he pretty much just lays down and on his side and hasnt ate (at least havent seen him eat). He appears to be breathing a little heavy but looking at the videos on this site not as bad as some show so im not sure if he is just stressed and to give him time or if there is more to it. his red color has faded a bit as it was more a solid maroon when i got him. I have attached a video which i hope loads as it is my first video post.

Also to add
Temp - 78
Salinity - 35 ppt
PH - 8.2
Alk - 10 (IO always seems to mix high)
Ammonia - 0
Nitrites - 0

That behavior is normal for a wrasse in QT, but the heavy breathing is not unless he recently exerted himself.

Any meds in the water? If not, I would give him a 5 min FW dip to check for flukes: https://www.reef2reef.com/threads/freshwater-dip.248898/

No meds, did fresh water just now and saw nothing. Did it look like heavy breathing to you in the video? new to all this and seeing some other stuff i have learned what i thought was something was no where need what various videos show it to be so just want to make sure.

Yes, it does appear something is causing his breathing to be labored.

How is he looking now? Still breathing heavily?

Seems to be worse off after the dip than prior. breathing seems more labored as well.

A freshwater dip can cause some odd behavior in fish right out of the dip. They can sometimes breath very rapidly and list a little when trying to swim. This usually goes away fairly quickly.

Did you find anything in the dip water afterward? Any flat, opaque, sesame seed looking deals in the water?

Nope nothing found, all looked clean.

No new change, still alive but laying on his side with rapid breathing. any thought on where to go from here? I have some API general cure not sure if thats worth a shot

At this point I would assume something parasitic (ich, velvet) or environmental (ammonia, low oxygen, outside toxin) is responsible for the heavy breathing.

If the QT has plenty of gas exchange & zero ammonia, I would begin raising copper: https://www.reef2reef.com/threads/copper-treatment.193343/

Notice something stringing out from the gill area and has almost like a chunk of meat on the end look. I originally thought it was a piece of brine shrimp just laying next to him but appears it is attached

That's not good. Could be excess mucous coming out, which forms when parasites start feeding and tissue becomes irritated.

So what would you say is best course of action for this? currently in general cure but also have coppersafe, prazi, kanaplex, focus, metroplex on hand

I did just pull the stuff off with tweezers and did appear to be mucus with just some food and partials trapped within it

Fairy wrasses sleep in a mucous cocoon at night. Looks like a bit of it was stuck to him. Is the fish still not eating? Is he otherwise alert? I've had fairy wrasses act this way for several days upon introduction to QT, but they will usually eat a bit when fed.

still not eating or really moving, looks similar to first video. he is attempting to stay more upright vs on side at time. used a acrylic tube to drop food right in front of him but neither the rod or the food made him move. I have had fish just lay there as well but if went near they would take off. i could reach in and grab this one if i wanted to. If turn the power head on he will float as if it were a dead fish

Prognosis isn't good based on that description. Personally, I'd try a round of broad-spectrum antibiotics (kanamycin or nitrofurazone) to see if it makes a difference, but usually when they reach the point of apathy/listlessness, they are too far gone.

I will see if i can find anything with either of those. initial search didn't provide any specific types so if there is anything specific you might recommend. I did just see him at least attempting to swim which is more than i have seen in a few day

ahhh i guess i have some kanaplex. Does anyone know if that can be used with general cure
