How did its anal fin get all shredded like that? What other fish are in with it? Are you seeing any aggression?

no aggression, only a Midas blenny in with the Naso. This is the bacterial spot that seems to have spread to the fin. Happened in a day or two.

Ouch on the heavy bacteria. Qurantine tank treated with seachem kanaplex will be needed

If using Neoplex- stick with it. It is broad spectrum

Dosing antibiotics without a sensitivity study is always just a guess. In this case though, the fish is getting much worse on the Neoplex, so I would switch to kanaplex. That is dosed more frequently, every 3 days.
