New clowmfish problems! URGENT!! HELP!

Will attach water parameters in a sec.

New common clown, only about 48 hours in the tank. (5gal, cycled)

We went out trick or treating with the kids, the clown started behaving a bit weird. Fast forward an hour later and he's lying on the ground, breathing heavily.

He was fine this morning, ate some flakes.

WHAT HAPPEND??

Water parameters:

Ammonia: 0
Nitrite: 0
Nitrate: 1-2
Salinity: 1.026
Temp: 26c (no change in past month.)

Took him out, seems a lot more pale then before. I thinks he's dead now anyway. Any reasons why he died?

Sorry for your loss. Did you test ammonia? Even cycled, 5G is such a tiny amount of water I’m sure the bio filter is fragile. Do you have adequate flow/oxygenation? At a minimum I would fresh water dip the corpse just to rule out flukes.
 
What do you mean by fresh water dip?

Ammonia : 0ppm

We also have polyfilter in the tank just in case of this.

How have you cycled the tank and how did you confirm the cycle? As @ariellemermaid says 5 gallon is very low amount of water, any drugs or chemicals in the water?

We finished the red sea reef mature program, and after that was a week of dosing sachem stability. Ammonia is at zero. And we also have polyfilter. The water in the tank in RO

No other chemicals.

I mean any time you have a dead fish put it in tap water. Look for any new spots that appear/drop off. Also have a look at the gills as well. That’s about as much post mortem as most folks can do.

Is this the first fish or are there others in the tank?

Only living thing, no coral.

DKH- high and may have impact on oxygen

Why on earth is the Alk so high? In and of itself though, probably not the culprit. Let’s talk oxygenation. What do you have for filter and flow? What brand tank are you using?
 
This sounds like an oxygen issue but another cause could be acclimation problems. When was the fish added and how did you acclimate? Did you check the salinity in the bag?

The tank is the Irish version of a petco tank. Filter is an ehim, will check want type in a moment.

Acllimation:
1hour floating
Added a small amount of water from tank to bag
15min
Let fish into tank

Did not check the bag water parameters but it's a good store, they won't sell you fish till you bring a water sample and a picture of your tank.
