New clowmfish problems! URGENT!! HELP!

What you’re looking for is good surface agitation constantly. Saltwater holds less oxygen compared with freshwater. Probably not hard to do in a 5g tank though, but we need to establish the facts of the tank. 1hr float is way too long; 15-30 min is all you need for the temp acclimation part. Still, I doubt acclimation is the problem for a fish that lasted 48H.
 
This sounds like an oxygen issue but another cause could be acclimation problems. When was the fish added and how did you acclimate? Did you check the salinity in the bag?

Added a small amount of water from tank to bag

Did not check the bag water parameters but it's a good store, they won't sell you fish till you bring a water sample and a picture of your tank.
I'm sure it's a good store. I always check salinity in the bag just to be on the safe side just to make sure I know what I'm dealing with when acclimating. Having said that if you followed their guidelines then it's most likely not that. BTW: I am so sorry for your loss! Hopefully the others can think of something like if high alkalinity is affecting other things like oxygen.
 
So what should I add to the tank?
It depends. If the flow is good and there’s lots of surface agitation then oxygenation probably isn’t the problem. I mean you can always add an air stone but the issue with that is crazy levels of salt creep.

I have to remain concerned about ammonia though if flow is ok. Brand new tank, first fish, etc. Can you provide a picture or video of the tank?
