I would examine leaving the tank without fish for a period of time to eliminate the velvet...

A little late catching up here.... Ended up pulling all my inverts and put them in another tank. Ran copper and everyone is healthy. 3 weeks and no signs of anything. I just added a humu-humu and a foxface. both are doing great.
No electrical issues. All my powerheads are brand new and cords are ran directly out of the tank - That is the only electrical in the tank. And since the copper everyone is happy and eating really well. My Niger trigger lost some of his fins but seems to be doing real well - eats, swims good, etc.

Cowfish was removed really early, while still sick and was in QT at LFS when he died. He was my daughters favorite, and would let her pet him.
