It would be great if could post a video - or post a descriptions of what you mean. The fish looks off - true - it could be old age or a neurologic issue. unless you have added more to the tank

Has fish been exposed to copper or high levels of carbon recently? If not , could be Hexamita especially if nitrate recently had increasedyes still eating, seems to be doing well

Nothing added for yearsthere is an obvious gill infection/injury - and to me it seems like there is a velvet-like 'clouding' on the skin. certainly that gill is not normal - it may be a tumor, or something else - if nothing else has been added to the tank.
My take on this is the fish is simply at “end of life”. The skin is dull and it is very thin. The material protruding from the gill is either a neoplasm or goiter….both are seen in long term captives. I do not see any treatable disease here…. Sorry.Nothing added for years

Could also be a concurrent Mycobacterium infection (another old fish disease). Since nothing has been added to this tank for many years and the other fish are ok, I ruled out protozoans and metazoans.I'm curious - and of course it's based on the photo - do you think there could be more there - i.e, cloudiness , etc. suggestive of something else?
