Post your parameters as well as age of the tank. Equipment used (lighting, flow, other equipment) And dosing anything?

Yes, that head is totally gone. Your salinity is a bit high. I’d bring it down to 1.025. Also 0 phosphate isn’t good. Could be starving him.

That’s food but they need some phosphates in the water as well. You want like around .02-.05, not 0.

Light is reef breeders the tank it was in had run only around a year or so the tank it's in now around 10 yrs I got it and just used the water from pervious owner

Reefbreeders light is fine. Torches like moderate flow. Powerheads should be in the torch direction but not directly at them. They should wave vigorously in the flow.

Flow is an icecap gyro it is on the other side of tank and hit it with I'd say perfect flow it looks like natural flow
How do I save it and make it grow the tissue on it is not good at all the guy I bought from ripped me off and I asked for my $ back and he said he would but never did so now I'm stuck with trying to get it growing and healed up

Good water quality, lighting and flow. The dead head will not regrow. Focus on keeping the good head alive.
