Correct me if I'm wrong but I believe velvet gives the fish more of a sheen along with the white specks but @Humblefish and @melypr1985 would know better.
Ich trophonts are larger (easier to see) and oval shaped; whereas velvet looks like tiny round specks all over the fish. The numbers game is another way to tell. Typically, if you can count the white dots on the fish its usually ich. However, if they are too numerous to count there's a good chance you are dealing with velvet.
