TTM how does anyone think this will work?

Exactly.

You can literally take an obviously infected fish from an LFS and do 4 x 72 hour rounds of TTM with no medication, and on day 13 it’ll be ich free.
 
I started TTM last night for 3 fishes with ich. What do you all suggest I do if they get an outbreak during the course of treatment? Stay the course, don't use any medication, and keep transferring every 72 hours?

If you are sure its ich (and not velvet), stay the course and keep transferring every 72 hours. You will eventually outrun the parasite's life cycle by Day 13 (at the latest).
 
So, I just completed the 4th transfer. I've been neurotic about sterilization and cross contamination. The threadfin butterflyfish, puffer and PBT have been looking good when viewing through the tank glass. I did, however, notice during today's transfer that there were some tiny little spots (not a lot) on the PTB. Should I expect to still see them on the fish at the 4th transfer? Thanks everyone for your insight and wisdom.
 
depending on how badly infected the PBT was, what you are seeing MAY be the scarring left over from where the parasite attached and embedded. By the 4th round, there should be zero attached parasites. at this point, id put him in an observation tank for 3-4 weeks to see if the spots go away as the fish slime coat recovers. However, any chance you contaminated the process- all bets are off. If hes looking otherwise healthy and active, and stress free, a 5th round might give you more piece of mind, but the spots will likely take longer than 72 hours to disappear, assuming they are scars.

you mentioned on the 6th of may that you started ttm.
first tranfer should be 72 hours later, 9th of may
second transfer on 12th of may
third transfer on 15th of may
fourth transfer on 18 of may
