Did you dip them yet?
I had an issue with a couple euphyllias last year, looks similar circumstance,
(NOT saying this is it, but a dip may help).
There was this white worm like creature burrowing & living in the branches, it would kill the heads, individually.
When i went to cut off the dead and dying heads / branches is how I found it. It scurried back into the colony.
I dipped them with bayer until i got them, I did end up tearing apart a branching hammer pretty bad, but it's not in my tank anymore.
Almost elucid worm looking like, bit stringy - er.
