Wrasse issue

I have a pintail wrasse who's been in my tank for a few months now. He was fine last night ate like a pig. This afternoon he's laying on the sand breathing heavy with a white ring around his head and gils. He was qted for 90 days no meds before added to my tank. What is it and what should I do? I cant find anything about a white ring like this
 
Is it possible to capture a pic? Would help... also do you have a way to isolate the fish in the tank? Like a nem/mushroom box?

is it poss he got head stuck in a outtake pipe,mouth of another fish/eel??
 
He's only in there with a two clowns, a lawnmower blenny, and a fire fish (they are all doing fine)

Also the overflows and returns on the nuvo 40 o think are to small for his head. Could he have gotten into a mp10??
 
I would think some kind of accident, just because when I blow the picture up that white looks like he's missing some scales/flesh in that area. But I suppose maybe a infection?
 
I have a few crabs, emerald green, scarlet reef, blue leg, electric blue hermit. But none look big enough to wrap completely around his head lime the white line
 
poss head stuck in empty snail shell,how is he doing now?
