I went for quite some time not QTing and had no issues, until I did and velvet wiped out all but 4 fish. I nursed them back to health and decided to QT going forward. 4 months later ich broke out and everyone back into QT and DT fallow for 6 weeks. Those who survived QT are back in the DT and doing very well. That being said, I have been struggling to get anything new through QT or Observation since. Everything keeps dying almost out of nowhere and I don't know why. I had a gem tang make it through QT. I put him into observation with an already QT'd koi scopas and lt tang. Each got a freshwater dip first to make sure no flukes. Gem was fine, went into DT and died within a day or two. Koi and LT showed maybe a little bit of flukes so back into QT with some prazi, airstone, powerhead, etc. They progressively got worst. Breathing labored and eventually they both died the exact same way it seemed. They were fine in observation for almost a month... I just don't get it. Then there were cheap LFS fish and they would seem fine for a couple weeks and then boom... dying off one at time at a fairly fast pace seemingly out of the blue. I had them in copper. They didn't even make it to prazi. Parameters appeared fine so no clue what happened. Anyhow, I have an IM 20g long peninsula for QT and an IM 30 gallon for observation. Both are cycling currently and I'm making sure they are pristine before bringing in anything else. I got new bigger airstones for each and added extra flow, filtration, etc. I've been following Jay Hemdal's method but I'm open to trying some other things due to my lack of success.

Sorry for your loss . One of my gem tang got ich pretty bad after stressful water change and another gem tangs bully. I just kept it wellfed and remained in DT. It recovered in a week. On the other hand PBT still covered with ich. So far hanging there . Other tangs either not affected or recovered. I dont quarantine new fish because there are ich and velvet already in my DT at the beginning of hobby . My tank maintenance guy brought me some fish directly from someone private house Which carried ich and velvets.
