Fish died

Any idea from the looks of him what he died from? Water parameters are good including salinity. 55 gallon tank with an amisojoy canister filter and protein skimmer.

Was he showing signs of distress? Eating? Other fish bullying?

How long was he in the tank?
 
He was alone in the tank. I have had him a month and a half. No signs of distress and ate this morning
 
You can buy an inexpensive voltage multimeter at Walmart. Test your equipment one by one. If you don't have a ground probe, it would be best to purchase one too.

Have you checked all your water parameters?
 
Yes all parameters are in normal range. Temp 79.8 degrees. I will definitely buy a voltage multimeter tomorrow
 
Her fins look messed up and the head is swollen but no idea if this happened after death.

This does not look like any result of stray voltage . Was fish eating up to the time of death ?
Was it breathing normal or labored that you saw ?
Was it getting lethargic and refusing to eat and hiding?
swimming normal or darting occasionally ?

Voltage isn’t generally a problem with fish… they are not grounded. Unless you had say a broken heater on one side and a pump on another. It’s sort of like how people in a pool don’t get zapped until they go to grab the metal railing to get out.
He has always had a swollen head. I got him from my work, they said he was born that way. They put him in a defective tank (because they couldn't sell him). He just died less than an hour ago because I had just turned off the light when I last checked on him

Swimming normally but right before I turned off his light he was swimming vertically. I didn't think anything of it because he usually sleeps like that, he has always breathed a little fast. He ate this morning but didn't eat when I fed him this evening.

Did colors seem a little faded?

well it is possible his deformity lead to his death. There is no telling if he had other internal defects.

Im suspecting Brooklynella which is a typical clown disease and to go into detail to assure you had the symptoms, fish may scrape up against objects, rapid respiration develops, and fish often gasp for air at the surface as the gills become clogged with mucus. Very quickly the fish will become lethargic, refuse to eat, and its colors will fade.
