Going Fallow After 5 months Due To Ich

Since you said some of the fish are still not eating, I would raise the Cu slowly. Take 3-4 days to get it up to therapeutic. Post closeup pics of the fish you think have a bacterial infection or Uronema so we can gauge the seriousness of it.

Here is the Anthia, doesn't look too bad. Kole tang losing color...he was bright blue 4 days ago....still not really eating his seaweed which he used to go crazy for. He is absolutely unhappy in QT, as well as the Starry Blenny who refuses seaweed too when he used to go crazy for it.

The pics you posted don't look bad at all. I think I would focus on raising the copper first and see what happens. If the infections are mild, the fish's natural immune system can usually deal with that on its own. Keep the antibiotics on standby just in case.
I had the same experience very recently and going through Qt atm. It was the first fish I added so losses weren't too bad but still, like you had to take entire reef apart.
Very similar symptoms also and I couldn't really diagnose Brook or urenoma, suspecting the latter then ich appeared.
Death of the first fish was pretty quick from ok in the morning, to hiding hiding, then dead in the evening.

.. Need to remove the carbon before copper treatment and u need to get a copper test kit as too much copper poisons them.

Update, about 8 days in....
Everyone is eating, except the male Anthia who mostly hides at bottom of QT and pokes his head out when food is introduced. Even then, he only has a small bite of brine shrimp which is sad as he used to be ravenous. I've tried garlic guard and I am soaking in Selcon.
He then retreats to bottom of tank and lies there. He will not eat live blackworms like he used to either. Not sure that it is necessarily related to copper as he hasn't really been ravenous since being moved from DT.
Copper has slowly been raised over 7 days to about 0.35. Will hold it steady here for next 30 days.
No ammonia spikes. Ammonia alert did hit 0.2 and has not gone back down, but I believe it takes a few days for the chemical reaction to change color back to "safe." I've done 10% water changes every 3-4 days to control ammonia.
DT has had a full cleaning. All equipment has come out and been cleaned. It looks amazing. There are literally hundreds of thousands Amphipods and Copepods crawling all over the glass, rocks and sand. Life looks good in there.
Letting ich's lifecycle run its course....

Update:
2 weeks QT done, 10 days copper. Everyone alive.
Algae starting to grow in QT.
No signs of ich. Some blotchy loss of color in royal gramma in small areas. Will try to send pic. Probably reaction to copper.

Update:
16 days into copper. Male Anthia passed yesterday. Did not do well in 0.35 copper. Possible had something going on internal as didn't have many signs of outward disease manifestation.
Kole tang, 2 stubby ocelaris, starry Blenny and Royal gramma all doing well. Eating happily.
Ammonia at zero.

Sounds like you have 2 weeks left to go with all the other fish.

Update:
30 days Copper complete. No signs of ich yet, have been observing for two weeks. All fish alive since last update.
Fish will need to remain in 20 gallon long QT for another 30 days while DT remains fallow.
Would anyone prophylactically treat with prazipro at this point? I think it's a good idea to ensure no parasites make it to DT. I can't really tell if any of the fish have internal parasites so I am tempted to just treat.
Also, if I do prazipro, and after finishing it, I think it would be a good idea to do a broad spectrum abx. Only reason being is that Royal Gamma has some loss of color of purple skin. Doesn't look overtly bacterial. He acts normal as well with good appetite.
Any thoughts?

I always prophylactically treat with prazi. Flukes can hide out in the gills as easily as ich can.
I would try soaking his food in fish vitamins (exs. Selcon, Zoecon, Vita-chem) or antibiotics (Kanaplex) before actually dosing such. I usually only advocate dosing antibiotics when the infection is serious, as they can be harsh on fish (appetite suppression, robs O2 from the water) and slow acting to boot.
However, this might also be one of those rare times where Melafix and/or Pimafix (natural remedies) would be useful for treating a mild infection.

Yesterday I noticed my water in QT became visibly cloudy.
Likely a bacterial bloom.
I did a 25% water change today. Replaced carbon filter on HOB filter. Fish seem to be doing fine regardless or water cloudiness.
There are no medications currently in the QT. Any other thoughts?

Any direct sunlight hitting the QT? That can initiate a bacterial bloom. I'm assuming you checked for ammonia?
