Kinda has a issue with swimming upright. He has been on occasion swimming on his side as well.
He almost looks like he's trying to either display or eat algae from the glass (which bristletooth tangs do a lot of).

Is this just when the mouth is open? Or all the time? Is the mouth open all the time?

It seems the mouth is open a decent amount. Thanks for chiming in friend.

I couldn't find the picture for reference, but does it look like there is something growing just inside his mouth? Like maybe his teeth are growing?

How long has it been that way and how long have you had him?

I've had him about a week and a half. This is the first time i noticed it. He is the only one in the tank.

Anything in there that he could bite that would possibly hurt him? No live rock right? (thinking of bristle worms)

If it is bacterial. What would you recommend? Also he went through a rally bath when she got here then straight to the qt

Well we ruled out the other possibilities which is good. I'm going with bacterial as well. I would start with kanaplex then add the others if needed.

Are you offering him enough food? It might have injured itself and got infected trying to search for food within the tank. I had this happen to me with my purple tang, tangs eat a lot.
I agree with above suggestion, try kanaplex but make sure if you start you complete the full course of 10 days, dont stop it earlier even if the fish improved from the first day.

So add kanaplex every 2 days...for 10 days correct?
