There's something I don't understand about the ich cycle ... If the fish remain healthy and do not allow the ich to attach to them why doesn't the ich eventually just die off?

It's because the ich is attaching to them, they are in the gills where it's easier for them to attach. It also happens to be out of view for you so people tend to think the fish dont have any symptoms. People who say their ich went away and hasn't returned.... their fish have ich still, but it's in the gills where they can't see it still breeding and living in the tank. As long as a fish in present the parasite's life cycle will continue.
