Contact you LFS for the Chaeto. Regular water changes plus the reactor will do the work. From your pictures I noticed that the Blue tang has Head and Lateral Line Erosion...
With your bioload in a 40 gallon you will always have high nitrates. That puffer alone will poop enough to raise your nitrates. You may be able to lower them but with a big puffer, 2 tangs and a handfull of smaller fish you'll have to just accept dirty water and keep up on the waterchanges.
I was told by my LFS that it can be caused by running carbon constantly. I have since removed the carbon and it has gotten better.
Rinse the carbon first and you won't have that problem.

So now I’m my take I only run throw away blue foam then a sponge block then bio balls. Along with a protein skimmer
