I know this sounds silly but sometimes blue tangs will literally dive bomb into a hole and scrape themselves so it could be that....

I'm glad he's improving... but dont stop adding the selcon to his food. Using the best quality carbon is a good idea for a fish like this that is dealing with HLLE
