This is what I would do.... A freshwater dip to check for flukes. His scratching on the rocks is a red flag for me. These holes or scratches could be a bacterial infection that would be secondary to a parasite like flukes or ich.
Depending on the results of the dip, the treatment instructions will be a bit different.

It probably does have HLLE but I agree that there is most likely a parasite issue going on here - ich being in the forefront of possibilities since flukes were negative.
