I've seen that happen to them after a water change with water that has salinity and temp that doesn't match current

I have 2 Mars aqua black boxes! I run the whites at 30% 7 hrs a day and blues at 50% for 9-10 hrs a day.

Initial thought is not enough light. Has he been moving around?

If it's stressed don't feed it. Other than the tentacles it actually looks fine on closer inspection. What makes you feel it's not well?

To me it looks like a lot of its tentacles are gone and shrunk up a lot less tentacles than before

Too much light or not used to your lighting yet. You have color and you said it is expelling stuff everyday and Tentacles or short. This is usually a sign of too much light as it is expelling its Alae to try and compensate for it. Maybe try not feeding for a week or two. A sign of not enough light is long Tentacles and pale color.

Oh yea that is definitely not good looking. Stop feeding it to start. What are all of your parameters? Any ammonia? Have your parameters been stable? Any large swings in ph, alk, etc?

A healthy nem will move itself away from too much light IME.

Also forgot to ask, could it have a torn foot? Did it get pulled off a rock or something?

That's what I was thinking too! If it's too much light wouldn't he move away from it?
