X2 that's velvet and significant gill damage... get him in copper ASAP. I wouldn't be surprised if he doesn't make it, I would freshwater dip this fish for immediate relief but it's probably too late. The lack of a stomach is not good either, looks like he hasn't eaten in weeksYour tang looks "roughed up", like when a fish has suffered damage from velvet. I'm also pretty sure he has a bacterial infection.

At the very least you'll want to use copper and a wide spectrum antibiotic on him. (See blue links.)
